The patient’s memory

With each validated document, Doctalis adds to the memory the facts it gives, each with that document as its source, in fixed categories: allergies and intolerances first, then current treatments, personal and family history, way of life and other. A document imported into the record feeds it the same way. A synthesis of a few sentences, each with its sources, is written again with each document validated, imported or deleted.

A fact is corrected or removed from its menu, and “Add a fact” adds one with you as its source. Each change is kept with who made it; removing a fact does not change its source document.

When two sources contradict each other, the contradiction shows at the top, both statements side by side, and a treating practitioner settles it. The memory ranks nothing by clinical risk and raises no alert.
